A fetotomy, partial or complete, can be performed on both the standing or recumbent cow. Both positions have advantages and disadvantages, and sometimes there is no choice due to available, or lack thereof, facilities or the condition of the cow. From the obstetrician’s point of view the standing position is far more convenient but is less safe for the cow. If the cow is sitting in sternal recumbency and unable to rise, elevation of the hindquarters will be helpful initially. As the procedure progresses, access to the fetus becomes easier with the cow in complete lateral recumbency. Alternatively, she can be placed in left lateral recumbency right from the start, with the first part of the fetus to be amputated situated uppermost. In this position the rumen will not be pushing the uterus down. In order to amputate the other extremity, the cow should be rolled over on to her right side so that part of the fetus is now uppermost. This manoeuvre may need to be repeated if a total fetotomy is to be performed.

One of the major problems during a fetotomy arises from breaking of the wire. When threading the wire, it must be handled very carefully, especially avoiding it from getting kinked. Such kinks produce weak spots at which the wire may break when sawing. Fetotomy wire, which is multifilament in design, tends to curl very easily, even after one or two cuts. Therefore it is important to examine the wire after each cut and, if necessary, to replace it if it is kinked or frayed. To lessen the chance of breaking the wire when sawing, it is advised to rotate the wire through the fetotome as each cut is made. A new length of wire should be used for each fetotomy procedure.
