• Patient-level percentage of bleeding on probing (BoP) （>25% high risk, <10% low risk）
• Prevalence (number) of residual pockets ≥4 mm following active periodontal therapy(>8個 high risk, <4個 low risk)
• Loss of teeth from a total of 28 teeth（>8顆 high risk, <4顆 low risk）
• Loss of periodontal support in relation to the patient’s age（low< 0.5 <moderate<1 high）
• Systemic and genetic conditions(沒有或不知道是low, 有的話是high)
• Environmental factors such as cigarette smoking.(不抽、已戒五年是low, 每天20根以上是high,偶爾抽＆每天11~19是moderate)
